STUDENT AFFIDAVIT
COURSE DESCRIPTION & NATIONAL EXAM
AUMT Medical Assistant Program is designed to prepare a student to become a Medical Assistant with a short yet
comprehensive course of study. This fast-track training prepares the student to sit for the Medical Assistant
(CCMA) national certification exam offered through the National Healthcareer Association (NHA) using a variety of
classroom and externship training experiences.
Student must complete 200 hours of didactic training and pass the class with accumulative score of 75% or higher.
Students who passed the 200 hours of didactic training will be allowed to take the National Exam at AUMT.
Students will receive a voucher from AUMT to register online www.nhanow.com for the national exam.
CLINICAL EXTERNSHIP TRAINING
Students are required to complete 160 hours of clinical training. Clinical sites are determined for each student by
the externship coordinator, and each student must attend his/her assigned site. Every effort will be made to ensure
that the externship site is within 50 miles of the school. Clinical externship is scheduled during regular weekday
working hours (usually 9:00 AM – 6:00 PM, M-F), students must make arrangements to complete their clinical
training during the day. It will take approximately 4 – 8 weeks to complete training.
TUITION POLICY
The program cost is the total charge to the student, which includes tuition, application fee, and course materials
fees. Total charge of the program is due and payable on the first day of the class unless other arrangements have
been made. By completing registration for a semester or session, students become financially responsible for
payment of all applicable fees by the due date(s). Payments not received by the due date(s) are subject to
collection, attorney, and litigation costs, which also become a financial obligation of the student.
Additionally, accounts must be current to maintain enrollment eligibility and to request and receive official school
documents and services. A "returned check charge" of $25 is assessed for a check returned by the bank for any
reason. Under California Civil Code #1719, a returned check may create a liability of treble (three times) the
amount owed, but not less than $100.

GRADUATION
I understand that in order to qualify for a Certification, I must meet the following requirements:
1. Achieve a cumulative grade point average of 2.3 or higher (grade of C or higher)
2. Attend at least 100% of the scheduled lecture hours
3. atisfy all financial obligations to the school
4. The student must satisfactorily demonstrate the knowledge and skills required by the occupation for which
the training is intended
