STUDENT AFFIDAVIT
DIDACTIC TRAINING, NATIONAL EXAM, REFRESHER
Students must complete 40 of the 48 hours of the didactic training and pass the class with a cumulative score of 75% or higher.
Only students who passed the didactic training will be allowed to take the National Exam.
Students must pay $129.00 online at www.nhanow.com to register for the National Exam in advance.
AUMT does not offer Continuing Education credits.
CLINICAL EXTERNSHIP TRAINING
All 80 Hour students are required to complete 40 hours of clinical training. Only students who pass the National Exam will be placed in a clinical training site. Sites are determined for each student by the Externship Coordinator, and each student must attend his/her assigned site. Every effort will be made to ensure that the Externship site is within 50 miles of the school. Clinical externship is scheduled during regular weekday working hours (usually 9 am - 6 pm, M-Fri). Therefore, Evening and Saturday students should make arrangements to complete their clinical training during the day. It will take approximately 4 to 8 weeks or more to place students in an externship.
COVID-19 EXTERNSHIP TRAINING UPDATE
Due to the recent outbreak of COVID-19, many of our externship programs were canceled or postponed until further notice. As a result of this pandemic, it could take approximately 10 – 20 weeks or more to place students in an externship. However, the student could be placed earlier than the anticipated date as we are making every effort to reduce the wait time.
TUITION POLICY
The program cost is the total charge to the student, which includes tuition, application fee, and course materials fees. The total charge of the program is due and payable on the first day of the class unless
other arrangements have been made. If your NHA study guide is lost, damaged or stolen, you may purchase a replacement for $40. Other costs that are required in obtaining a California CPTI License are not included in the tuition fees. By completing registration for a semester or session, students become financially responsible for payment of all applicable fees by the due date(s). Payments not received by the due date(s) are subject to collection, attorney, and litigation costs, which also become a financial obligation of the student.
Additionally, accounts must be current to maintain enrollment eligibility and to request and receive official school documents and services. A "returned check charge" of $25 is assessed for a check returned by the bank for any reason. Under California Civil Code #1719, a returned check may create a liability of treble (three times) the amount owed, but not less than $100.
CANCEL, WITHDRAWAL, TERMINATION, REFUNDS, REINSTATEMENT
No refunds will be issued under any circumstances after the start of the first class meeting. However, the student may request a refund if the request is received no later than 3 full business days before the first class meeting. There will be a $10 service fee. The school has the right to withdraw/terminate a student for the following reasons and no refunds will be issued:
Not meeting minimum attendance requirements.
Not complying with the Externship Policy.
Not completing the program within the Scheduled Completion Date.
Not passing the class with a cumulative score of 75% or higher

GRADUATION
I understand that in order to qualify for a Certification, I must meet the following requirements:
1. Achieve a cumulative average grade of 75% or higher.
2. Attend at least 83% of the scheduled lecture hours and 100% of the Externship hours.
3. Satisfy all financial obligations to the school.
4. The student must satisfactorily demonstrate the knowledge and skills required by the occupation for which the training is intended.
